Frequently Asked Questions
You can apply or reapply for OHP on the phone or in person. You can call OHP at 800.699.9075 or visit a community partner at Klamath Open Door.
You may also apply online or through your ONE account.
CHA members can get free rides to and from scheduled non-emergency appointments and wellness classes through the Translink service.
Please call Translink at 541.842.2060 to schedule a ride at least 48 hours in advance. Rides are available Monday through Friday, 7 a.m. to 5 p.m.
Please do not pay it. Call your provider's office instead. Make sure they have billed CHA using your member ID number. If you still get a bill, call CHA at 541.883.2947 and will assist you.
A CCO is a health plan that brings physical health, mental health, dental health and other services together in one group. CHA is a provider-owned CCO based in Klamath Falls, Oregon. We work with our members to give them better health for a better future.
CHA customer service can send a new insurance card to the address listed on file for you, or you can stop by our office Monday through Friday, 8 a.m. to 5 p.m., and pick one up.
We are located at 2909 Daggett Ave., Suite 225, in Klamath Falls.
You should have received a copy of the member handbook within 30 days of enrollment with CHA. If you need a new one, you can stop by the CHA office to pick up a copy or request a copy in the mail by calling us at 541.883.2947.

Take a look at our provider directory to learn if your physician or dentist is covered. We have a large network, so it is likely your doctor or dentist is covered. If not, you can call customer service Monday through Friday, 8 a.m. to 5 p.m., at 541.883.2947. We can help you choose a primary care provider or a primary care dentist who is in our provider network.
You can change your primary care provider once every six months. To change your provider, please call CHA customer service at 541.883.2947.
Members who are pregnant or age 20 and younger who have medical coverage through CHA are eligible for routine eye exams and vision correcting services.
If you have dental coverage through CHA, basic services—including cleaning, fillings and extractions—are covered.
Yes, we do. If you have dental coverage through CHA, full dentures are paid for once every 10 years and partial dentures once every five years.
Yes, we do. Getting a flu vaccine is an important way to stay healthy.
Members age 19 and under can get flu shots at your primary care provider's (PCP) office. Members age 20 and above can get flu shots at your PCP's office or your pharmacy.
To change your contact information, you need to contact the Oregon Health Plan directly. You can do this in one of three ways:

- Email oregonhealthplan.changes@state.or.us.
- Log into your ONE account.
Congrats on your decision! We are happy to help you improve your health. CHA offers six smoking cessation courses per year. Each course includes one class per week over seven weeks.

Your case manager can help you arrange your prenatal care and enroll you in our Maternity Incentive Program.
