Covered Medications

The following medications do not require a Prior Authorization for the first 30 days when utilized for Medication Assisted Treatment (MAT):

  • Suboxone tablets and films (generic)
  • Subutex tablets (generic)
  • Vivitrol injections
  • Sublocade injections

If Your Medicine Isn't Covered

If your doctor thinks the medicine is the best option, he or she can contact us. Doctors must fill out a prior authorization request form for coverage.

Your doctor must include all supporting documentation with the request form. CHA's clinical team will then review the request. They will approve it if they believe there is a medical need. In some cases, the team may recommend a different drug. They may also deny the request if they feel it isn't medically appropriate.

Our Medication Coverage Policies

We use the CHA drug formulary to make sure you get the best medicine possible. This is a list of covered medicines chosen by the CHA pharmacy and therapeutics committee. The goal of the formulary is to help providers choose the most cost-effective drug therapy for our members.

CHA members get either a 30- or 90-day supply. Most of our medicines are approved for 30 days. Some prescriptions may be extended to a 90-day supply.

Medicine to Treat Hepatitis C

Providers who want to treat a member for hepatitis C must fill out our Hepatitis C Treatment Request Form.
